Job summary

Southcoast Health is seeking a Licensed Practical Nurse for Medical Surgical - VNA to deliver in-home and community-based care across our visiting nurse areas. Per diem day shift coverage is required, serving the Fairhaven location and surrounding communities.

The position emphasizes patient education, health maintenance, and teamwork to provide high-quality care in diverse home settings. MA LPN licensure is required, with BLS/CPR certification.

Qualifications

• Graduation from an accredited school of practical nursing is required.
• One year of nursing experience is preferred.
• Current LPN licensure in MA (and possibly RI depending on geographic area).
• BLS & CPR certification is required.
• Proficient computer skills, including experience with various software and electronic records.

Responsibilities

• Under the direction of the Clinical Nurse Manager, provide competent health care to individuals and families in the clinical setting.
• Promote health maintenance through teaching, counseling and preventive measures.
• Adapt care plans for home and community-based settings.

Hours: Per Diem

Shift: Day shift, 8:00am - 4:30pm

Location: Visiting Nurse Association - Fairhaven, MA; Team 1 coverage area- Rehoboth, somerset, Swansea, Seekonk,Assonet, Raynham,Taunton,Dighton, Berkely and Freetown.

Promote health maintenance of individuals, families and the community through teaching, counseling, collaborative planning and appropriate preventative and rehabilitative nursing measures.

• Graduation from an accredited school of practical nursing is required.
• Proficient computer skills, including experience with various software, database and electronic records programs is preferred.
• One (1) year of nursing experience is required; home care experience is preferred.
• Current LPN Licensure is required in the Commonwealth of Massachusetts and may be required in Rhode Island, subject to geographic area assignment.
• Home care experience preferred
• Basic Life Support (BLS) & CPR certification is also required.
• Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired

Compensation: Pay rate will be determined based on level of experience.
