Job summary

Southcoast Health is seeking a dedicated LPN Staff member for Primary Care Swansea, MA. This 24-hour weekly schedule features day shifts with rotating start times, and includes a $3,000 sign-on bonus for new hires. Join a not-for-profit system that values compassionate, community-focused care.

You will perform direct patient care, administer medications, and teach patients and families under EHR guidance, while collaborating with a skilled care team in a supportive environment.

Qualifications

• Graduation from an accredited school of practical nursing or equivalent is required.
• Experience working with Electronic Health Record (EHR) is highly desirable.
• One year or more experience in related setting preferred.
• Current LPN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is also required.

Responsibilities

• Perform direct patient care, including medication administration and patient teaching, within the scope of practical nursing.
• Utilize an Electronic Health Record (EHR) and related software to update or access patient information.
• Ensure care is appropriate to the age group served and collaborate with other care team members to maintain professional nursing standards.

*$3,000 new hire sign-on bonus for this position. (rehires termed more than 1 year also eligible)*

Hours:24hrs

Shift:Day shift; rotating start times 8:00am - 4:30pm and 8:30am - 5:00pm

**Available to regular status employees who are scheduled to work a minimum of 24 hours.

Position reports to the Director, Manager or designee. Perform a variety of duties involving direct patient care, such as medication administration, patient and family teaching, and testing/procedures in accordance with organizational policies, procedures, and protocols and within the scope of practical nursing. Utilize an Electronic Health Record (EHR) and other related software to update or access patient information as appropriate. Has knowledge and skills necessary to provide care, which is appropriate to the age group of patients regularly served. Works cooperatively with ancillary nursing and other patient team personnel in maintaining standards for professional nursing practice.

• Current LPN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is also required.
• Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
• Compensation: Pay rate will be determined based on level of experience.
