Job summary

Southcoast Health in Swansea, MA is seeking a skilled LPN Staff to join our primary care team. The role involves direct patient care, medication administration, and patient family education in a supportive, community-focused setting.

24-hour weekly schedule with day shifts and rotating start times. Qualifications include graduation from an accredited practical nursing program, MA/RI LPN licensure, BLS certification, and EHR experience encouraged.

Qualifications

• Graduation from an accredited school of practical nursing or equivalent is required.
• Experience with Electronic Health Record (EHR) is desirable.
• One year or more experience in related setting preferred.
• Current LPN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is required.
• Must be fully vaccinated against seasonal influenza and COVID-19 or approved exemption.

Responsibilities

• Provide direct patient care including medication administration.
• Educate patients and families on care plans.
• Perform testing and procedures per organizational policies.
• Maintain accurate documentation in the Electronic Health Record (EHR).

*$3,000 new hire sign-on bonus for this position. (rehires termed more than 1 year also eligible)*

Hours:24hrs

Shift:Day shift; rotating start times 8:00am - 4:30pm and 8:30am - 5:00pm

**Available to regular status employees who are scheduled to work a minimum of 24 hours.

Position reports to the Director, Manager or designee. Perform a variety of duties involving direct patient care, such as medication administration, patient and family teaching, and testing/procedures in accordance with organizational policies, procedures, and protocols and within the scope of practical nursing. Utilize an Electronic Health Record (EHR) and other related software to update or access patient information as appropriate. Has knowledge and skills necessary to provide care, which is appropriate to the age group of patients regularly served. Works cooperatively with ancillary nursing and other patient team personnel in maintaining standards for professional nursing practice.

• Graduation from an accredited school of practical nursing or equivalent is required.
• Experience working with Electronic Health Record (EHR) is highly desirable.
• One year or more experience in related setting preferred.
• Current LPN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is also required.

• Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

• Compensation: Pay rate will be determined based on level of experience.
