Lebanon Center for Rehabilitation and Healing is seeking an organized and detail-oriented Licensed Practical Nurse (LPN) to serve as our Medical Records LPN. You will maintain complete, accurate resident health records in line with facility policies and applicable regulations.

The role blends clinical knowledge with strong administrative, organizational, and communication skills to support nursing staff and residents.

Qualifikationen

• Current, unrestricted LPN license in the state of employment.
• Experience with medical records and HIPAA-compliant documentation.
• Familiarity with electronic medical record systems and clinical documentation practices.

Aufgaben

• Maintain accurate, complete, and organized resident medical records.
• Review charts for missing or inconsistent documentation.
• Conduct routine chart audits and follow up with team members.
• File and process physician orders, lab reports, and other clinical documentation.
• Ensure records are updated promptly after admissions, transfers, and discharges.
• Process requests for medical records per HIPAA and facility policies.
• Safeguard resident privacy and confidentiality of information.
• Assist nursing and interdisciplinary staff with medical-record documentation needs.
• Prepare records for audits, surveys, and regulatory requests.
