Lebanon Center for Rehabilitation and Healing in Lebanon, TN is seeking compassionate Licensed Practical Nurses to join our clinical team. We offer full-time, part-time, and per diem roles with competitive pay and excellent benefits in a supportive rehabilitation and long-term care setting.

You will administer medications, monitor residents, provide direct nursing care, and collaborate with RNs and physicians to promote safe, resident-centered care.

Qualifications

• Current LPN license in Tennessee required.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Strong clinical and communication skills required.
• Compassionate, resident-focused approach to care.
• Ability to work effectively in a fast-paced environment.
• Team-oriented mindset and strong work ethic.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the nursing team.
• Provide direct nursing care and support to residents.
• Maintain accurate and timely documentation in medical records.
• Collaborate with RNs, physicians, and interdisciplinary team members.
• Assist with admissions, discharges, and resident assessments.
• Ensure compliance with facility policies and healthcare regulations.
• Promote a safe, clean, and comfortable environment for residents.
