Lebanon Center for Rehabilitation and Healing is seeking a detail-oriented LPN to serve as Medical Records LPN. The role focuses on maintaining accurate and confidential resident health records in line with HIPAA and facility policies.

The candidate should have an unrestricted LPN license and experience in medical records or health information. Familiarity with EMR systems and strong organizational skills are highly valued.

Formación

• Current, unrestricted LPN license in the state of employment.
• Experience in medical records or health information is preferred.
• HIPAA knowledge and confidentiality standards.
• Familiarity with electronic medical record systems preferred.
• Strong organizational and time-management skills.

Responsabilidades

• Maintain accurate, complete, and organized resident medical records.
• Review charts for missing, incomplete, or inconsistent documentation.
• Conduct routine chart audits and follow up with team members.
• File and process physician orders, laboratory reports, and other clinical documentation.
• Ensure records are updated after admissions, transfers, and discharges.
• Track required documentation, signatures, assessments, and care-plan materials.
• Process requests for medical records per HIPAA and policies.
• Safeguard privacy and confidentiality of resident information.
• Assist nursing staff with medical-record documentation needs.
• Prepare records for audits, surveys, reviews, and regulatory requests.
• Maintain archived records per retention requirements.
• Report documentation concerns to nursing leadership.
• Perform other nursing and medical-record duties as assigned.
