Job summary

Wellpath seeks Licensed Practical Nurse (LPN) to join our Chronic Care team. Under RN supervision you will contribute to assessment, care planning, and patient education, delivering care in our facilities.

You will administer medications, assist in procedures, participate in safety checks, and support a compassionate, patient‑centered approach. We offer ongoing training, competitive pay, benefits, and a mission to serve underserved populations.

Qualifications

• Graduate from an accredited school of nursing.
• Prefer a minimum of one (1) year clinic experience
• Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
• Must be able to obtain and maintain CPR certification.

Responsibilities

• Under RN supervision, assess patients' physical, psychological, and social needs.
• Administer medications, including IV therapy (if certified), and perform diagnostic tests.
• Assist in controlled substance counts at the beginning of each shift, adhere to safety techniques and standard precautions.
• Assist in medical or minor surgical procedures, implement nutrition and therapeutic diet plans, and document nursing encounters.
• Attend mandatory staff meetings and training sessions, demonstrating commitment to ongoing professional development.
