Job summary

Wellpath / Correct Care Solutions in Yakima, WA is seeking a Licensed Practical Nurse (LPN) to join our nursing team on a full-time Day shift, Monday through Friday. Under RN supervision, you will administer medications, provide patient education, and support care plans in our community healthcare facilities.

This position requires graduation from an accredited nursing program, active LPN licensure in the state, and CPR certification.

Qualifications

• Graduate from an accredited school of nursing.
• Prefer a minimum of one (1) year clinic experience.
• Have and maintain current licensure as an LPN/LVN in the state of employment; CPR certification.

Responsibilities

• Under RN supervision, assesses patients and participates in care planning.
• Administers medications, including IV therapy (if certified), and conducts diagnostic tests as required.
• Counts controlled substances at shift start and responds to health emergencies per protocol.
• Assists in procedures and documents nursing encounters using SOAP. Maintains communication with staff and physicians.
• Attends mandatory training and adheres to policies to provide high-quality patient care.
