Job summary
Genesis is seeking a dedicated LPN MDS Coordinator to support the Resident Assessment Process and ensure accurate MDS and CAAs for optimal patient care and center reimbursement.
You will lead the interdisciplinary RAI process, develop care plans, and train new team members on MDS content and electronic completion, while complying with Federal and State regulations.

Qualifications
- Licensed LPN in good standing with current licensure.
- Experience in hospital or long-term care setting preferred.
- Knowledge of nursing practices and regulations for long-term care.
- Must have basic Windows computer skills.
- MDS coordination experience and PPS/RAI knowledge preferred.
Responsibilities
- Direct the RAI process following guidelines for MDS completion, encoding and state transmission.
- Develop individualized resident Care Plans and identify needs and outcomes.
- Train new team members on clinical content and MDS item completion.
