Fundamental is seeking a The MDS Nurse (LPN) to coordinate the RAI process and ensure accurate MDS data for developing the interdisciplinary Plan of Care.

Qualifications

• Must have MDS/RAP experience and nursing knowledge.
• Must hold an unencumbered LPN license as required by state.
• Ability to utilize local and consultant resources in execution of responsibilities.
• Knowledge of state, federal, and regulatory requirements.

Responsibilities

• Coordinating the Resident Assessment Instrument (RAI) process
• Completing an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care
• Participating in the daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM)
• Establishing positive relationships with the healthcare team and nursing management
