Job summary

Genesis HealthCare is seeking an experienced LPN MDS Coordinator to oversee the Resident Assessment Process, ensure accurate MDS completion, and coordinate state transmission of data. You will drive the RAI workflow, help develop individualized Care Plans, and train team members on MDS items.

The role requires an LPN in good standing with long-term care experience, strong computer skills, and knowledge of MDS/Care Plans. Competitive hourly pay and benefits are offered.

Qualifications

Must be an LPN in good standing and licensed by the state.
Experience in hospital or long-term care setting required.
Knowledge of long-term care laws, regulations and guidelines.
Proficient with computer applications and MDS processes.
Experience as an MDS Coordinator and completing MDSs/Care Plans preferred.

Responsibilities

Direct the RAI process and MDS completion with state transmission.
Develop individualized resident Care Plans and identify needs and outcomes.
Train new interdisciplinary team members on clinical content and MDS items.
