Job summary

Edmonds Post Acute is hiring an MDS Assistant (LPN) in Edmonds, WA to support the MDS Coordinator by ensuring resident assessments are accurate, complete, and timely. You will apply your clinical knowledge to shape care plans, audit charts, and verify documentation aligns with regulatory standards.

This role focuses on documentation and data entry, with opportunities to learn the MDS process and grow toward becoming an MDS Coordinator.

Qualifications

  • Active Washington State LPN license (required).
  • At least 1 year of experience in long-term care or geriatric nursing.
  • Knowledge of RAI/MDS, PDPM, and payer sources (Medicare, Medicaid, HMO, private pay, hospice).
  • Comfort with EMR data entry; clerical experience is a plus.
  • Strong attention to detail and ability to meet deadlines.
  • HIPAA knowledge and commitment to resident confidentiality.

Responsibilities

  • Assist the MDS Coordinator with timely, accurate completion of MDS assessments under the RAI process, following state and federal regulations.
  • Help develop and update resident care plans in line with current RAI guidelines and required timeframes.
  • Maintain assessment schedules and track deadlines for MDS submission to the state.
  • Conduct ongoing chart audits to confirm documentation supports assessments and care plans.
  • Audit records for proper physician and family notification, and complete notifications when assigned.
  • Enter assessment data and documentation in our EMR.
  • Attend Interdisciplinary Care Plan meetings and talk with residents and families during care plan reviews.
  • Work with every department to make sure resident needs are captured and communicated.

MDS Assistant (LPN) | $35-$55/Hour | Edmonds Post Acute | Edmonds, WA

Edmonds Post Acute is hiring an MDS Assistant to support our MDS Coordinator in keeping resident assessments accurate, complete, and on time. You'll use your clinical knowledge to shape care plans, audit charts, and make sure every resident's needs are documented correctly. What You’ll Need
  • Active Washington State LPN license (required)
  • At least 1 year of experience in long-term care or geriatric nursing

  • Comfort with EMR systems and data entry; clerical experience is a plus

  • Ability to stand, walk, and move throughout the building, and lift up to 25 pounds
  • Annual TB screening and physical exam per facility policy
