Job summary

CenterWell Home Health in Baton Rouge is seeking a Patient Services Coordinator, LPN/LVN to bridge clinical staff and patients and their families. You will schedule and coordinate all aspects of a patient’s healthcare maintenance and treatment, following approved procedures and standards.

Qualifications

• Licensed Practical Nurse or Licensed Vocational Nurse license in the state of practice.
• 1 year of LPN/LVN experience.
• Preferred: 1 year of Home Health/Hospice experience.

Responsibilities

• Manage schedules for all patients, including supervisory visits.
• Schedule discharge visits, OASIS collection, recert visits, and Transfer to Inpatient Facility (TIF) OASIS collection visits.
• Reschedule declined or missed visits and process reassigned and rescheduled visits.
• Prepare and edit weekly agent schedules and review productivity/geographic issues.
• Assist with weekly case conferences.
• Receive calls from field staff and refer questions to Branch Director as necessary.
• May perform patient visits and participate in on-call rotation.
• Maintain the client hospitalization log and enter coordination notes.
• Send electronic logs to office staff and distribute lab reports as appropriate.
• Verify visit paper notes in scheduling console and run scheduling reports.
• Assist with internal transfer of patients between branches.
• Additional duties may vary by branch census and leadership discretion.
