Bethesda Health in Clayton, MO is seeking a dedicated Licensed Practical Nurse to provide basic nursing care, assess residents on admission and ongoing, and collaborate with the interdisciplinary team to ensure high-quality care.

You will document assessments, update care plans, administer medications and treatments safely, and communicate changes promptly using SBAR. IV care is performed only if IV certified and continuing education is required.

Competenze

• Licensure as an LPN from an accredited nursing school.
• Experience in long-term care, rehab, or assisted living preferred.
• Current LPN license in the work state.
• IV certification required or willingness to obtain within 6 months.
• CPR/AED certification required or willingness to obtain within 6 months.
• Ability to assess basic geriatric nursing needs.
• Excellent communication with residents, visitors and staff.
• Strong time management and organizational skills.
• Critical thinking skills.

Mansioni

• Provide basic nursing care and assess residents on admission and ongoing.
• Communicate changes to the IDT and update care plans.
• Document assessments accurately and timely.
• Administer medications and treatments safely.
• Provide IV care only if IV certified.
• Notify physician timely using SBAR.
• Monitor and adjust care per the care plan and QAPI processes.
• Assist residents with ADLs and respond to call lights.
• Complete 12 hours of nursing-related continuing education.
