Job summary

Bethesda Health Group is seeking a licensed practical nurse to provide basic nursing care and ongoing assessments for geriatric residents. You will document findings, communicate changes to the IDT and families, and participate in care planning with updated plans as interventions evolve.

Responsibilities include timely medication administration, potential IV care if certified, and accurate electronic and written documentation.

Qualifications

• Current LPN license in state of Illinois.
• CPR and AED certification required or willing to obtain within six months.
• IV certification required or willing to obtain within six months.
• Long-term care experience preferred.
• Ability to assess basic nursing care needs of geriatric residents.
• Excellent communication with residents, families and staff.
• Strong time management and organizational skills.

Responsibilities

• Provide basic nursing care and assess residents on admission and ongoing basis.
• Document assessments accurately and timely; communicate changes to the IDT and families.
• Participate in care planning and update plans as interventions change.
• Administer medications and treatments safely and timely; IV administration if certified.
• Maintain documentation electronically and in writing; use SBARR for physician communication.
• Support resident needs, respond to call lights, and assist with ADLs as needed.
• Support quality improvement processes and stay updated with Bethesda policies.
