Qualifications

• Active Illinois RN or LPN license.
• Missouri nursing license support and process assistance.
• Strong assessment and communication skills.
• Compassionate, dependable and self‑motivated attitude.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled, one‑on‑one nursing care in the patient’s home.
• Administer and manage medications as ordered.
• Monitor, document patient conditions and respond to changes.
• Perform treatments ordered by the physician.
• Educate and support family members; coordinate with the clinical team.
