Phoenix Home Care in Illinois offers private duty nursing designed for one patient at a time in the comfort of their home. You will provide skilled nursing care, administer medications, monitor conditions, and perform treatments as prescribed by physicians.

This role emphasizes compassionate, personalized care, ongoing family education, and close collaboration with our clinical team to ensure consistent, high-quality support for patients and their loved ones.

Qualifications

• Active Illinois RN or LPN license is required.
• Missouri nursing license costs covered and licensure process supported.
• Strong assessment and communication skills.
• A compassionate, dependable, self-motivated heart for serving others.
• Valid driver’s license and reliable transportation is required.

Responsibilities

• Provide skilled, one-on-one nursing care in the patient’s home.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.
• Educate and support family members.
• Collaborate closely with the Phoenix clinical team for ongoing guidance and support.
