Job summary

Phoenix Home Care and Hospice is seeking compassionate in-home nurses to care for one patient at a time in Jerseyville, IL. You’ll provide skilled nursing, manage medications, and document patient conditions in a private duty setting.

Qualifications

• Active Illinois RN or LPN license.
• Ability to obtain Missouri nursing license with support from employer.
• Strong assessment and communication skills.
• Compassionate, dependable, and self-motivated patient advocate.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled, one-on-one nursing care in the patient’s home.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.
• Educate and support family members.
• Collaborate with the Phoenix clinical team for ongoing guidance.
