Job summary

Phoenix Home Care & Hospice is seeking a dedicated nurse to provide skilled, one-on-one private duty care in patients' homes across Illinois. You will administer medications, monitor conditions, perform treatments, and educate families while coordinating with our clinical team.

We cover the Missouri licensure process and offer guidance and cost support. Benefits include medical, dental, vision, 401(k), PTO, life and disability insurance, plus weekly pay.

Qualifications

• Active Illinois RN or LPN license is required.
• Missouri licensure process covered / supported by employer.
• Strong assessment and communication skills are essential.
• Compassionate, dependable, and self-motivated attitude.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled, one-on-one nursing care in the patient’s home.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.
• Educate and support family members.
• Collaborate with the Phoenix clinical team for guidance and support.
