Qualifications

• Active Illinois RN or LPN license is required.

Responsibilities

• Provide skilled, one-on-one nursing care in the patient’s home.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.
• Educate and support family members.
• Collaborate with the Phoenix clinical team for ongoing guidance and support.
