Phoenix Home Care & Hospice is seeking skilled in-home nurses to provide one-on-one care in Illinois homes. You will administer medications, monitor conditions, and document patient progress while building meaningful connections with patients and families.

Qualifications

• Active Illinois RN or LPN license.

• Valid driver’s license and reliable transportation.

• Provide skilled, one-on-one nursing care in the patient’s home.
