Job summary

Phoenix Home Care & Hospice is seeking RNs or LPNs to provide one-on-one private duty nursing in patients’ homes in Missouri, with licensure sponsorship from the company. You’ll slow down the pace, build meaningful relationships, and deliver compassionate care.

You will administer meds, monitor conditions, educate families, and work closely with a clinical team to ensure high-quality, patient-centered support in a home setting.

Qualifications

• Active Illinois RN or LPN license.
• Strong assessment and communication skills.
• Compassionate, dependable and self-motivated.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled, one-on-one nursing care in the patient’s home.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.
• Educate and support family members.
• Collaborate with the Phoenix clinical team for guidance.
