Job summary

Mental Health Association of Rochester seeks a Licensed Practical Nurse (LPN) to provide skilled nursing care in a long-term care setting. You will assess residents, administer medications, monitor responses, and document outcomes for continuous improvement.

You will work with a multidisciplinary team, ensuring confidentiality and safety while supporting residents and families with preventive plans and ongoing self-care education.

Qualifications

• Graduate from an accredited practical nursing program; associate degree preferred.
• Valid unencumbered LPN license with state nursing board or multi-state licensure.
• Experience in clinical settings, including supervised clinical hours or minimum 400 hours as an LPN.
• For IV therapy, completion of approved training or courses as required by the state.
• Required continuing education and competency certifications, with ability to work within interdisciplinary teams.
• Strong communication skills, attention to confidentiality, and ability to work effectively with residents and healthcare staff.
