Maintain residents' health, comfort, and safety while working within an interdisciplinary team. Provide technical support and communicate effectively with residents, families, and healthcare providers.
Graduation from an accredited nursing program, current LPN licensure, preferred experience in long-term or sub-acute care. Strong communication, confidentiality, and problem-solving skills, with the ability to work collaboratively in a team environment.
Deliver holistic resident care, ensure safety, document interactions, and respond effectively to emergent situations. Support residents' psychosocial and physical needs, following care plans and standards of practice. Perform additional related duties as assigned.
