Job summary

Accela Post Acute Care at Hamilton in Hamilton Township, NJ is hiring a Licensed Practical Nurse (LPN) to join our resident-focused team. You will administer medications, monitor health status, and help develop individualized care plans in a 55-bed post-acute setting on the campus of RWJ University Hospital-Hamilton.

The ideal candidate holds a New Jersey LPN license, has LTC or acute care experience, and demonstrates excellent English communication, professionalism, and teamwork.

Qualifications

• Current LPN license in New Jersey.
• Experience in Acute care LTC or similar settings.
• CPR certification preferred.
• Effective verbal and written English communication skills.
• Highest level of professionalism with the ability to maintain confidentiality.
• Ability to communicate at all levels of the organization and work well within a team.

Responsibilities

• Provide compassionate and personalized care to residents, ensuring comfort and well-being.
• Collaborate with the healthcare team to develop and implement individualized care plans.
• Administer medications, monitor health status, and provide education to residents and families.
• Foster meaningful relationships with residents and their families, creating a warm and welcoming environment.
• Participate in activities and events that promote a positive atmosphere and enhance residents’ lives.

Licensed Practical Nurse Responsibilities Include:

Weekend Requirement:

This position requires rotating weekends (on/off schedule). All Per Diem staff must commit to working at least one full weekend per month (both Saturday and Sunday).
