Accela Post Acute Care at Hamilton in Hamilton, NJ seeks an LPN Licensed Practical Nurse to provide compassionate, resident-centered care on a rotating weekend schedule. You will administer medications, monitor health status, and collaborate with a multidisciplinary team to create and implement individualized care plans.

Qualifications

• Active LPN license in New Jersey.

• Team-oriented with ability to work across organizational levels.

Responsibilities

• Provide compassionate and personalized care to residents and monitor health status.
• Collaborate with healthcare team to develop and implement individualized care plans.
• Administer medications and educate residents and families.
• Foster a warm, welcoming environment and meaningful resident interactions.
• Engage residents in activities and events to promote a positive atmosphere.
• Coordinate with on-call staff for weekends and shift coverage.

Licensed Practical Nurse Responsibilities Include:

• Ability to communicate at all levels of organization and work well within a team environment in support of company objectives
• Customer service-oriented with the ability to work well under pressure

Weekend Requirement:

This position requires rotating weekends (on/off schedule). All Per Diem staff must commit to working at least one full weekend per month (both Saturday and Sunday).
