Job summary

The Grand Rehabilitation and Nursing at Queens in New York is seeking a full-time Licensed Practical Nurse (LPN) for our Skilled Nursing facility. The ideal candidate will provide patient care, administer medications (including IVs), monitor vital signs, and collaborate with physicians, NPs, families and CNAs.

Shifts available 7am-3pm and 3pm-11pm; NY license required; experience in geriatric care preferred.

Qualifications

• Active NY LPN license.
• Experience in SNF/long-term care preferred.
• Ability to work 7am-3pm or 3pm-11pm.

Responsibilities

• Provide care for residents in Skilled Unit and Long Term Care Unit.
• Medication administration, including IVs.
• Take and record vital signs and monitor outcomes.
• Educate residents and families; communicate with physicians, NPs and CNAs.
• Supervise CNAs and coordinate with health professionals.

Candidate should possess passion for geriatric population, must contain a license in the state of NY, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience. Rate: Up to $34.48 an hour
