The Grand Rehabilitation and Nursing at Queens is actively seeking a full-time Licensed Practical Nurse (LPN) to join a dedicated geriatric care team in a Skilled Nursing Facility. Candidates must hold an NY LPN license and be able to work 7am-3pm or 3pm-11pm shifts.

Hiring immediately with daily pay options and a range of benefits. Responsibilities include medication administration, vital signs, resident education, and collaboration with physicians, NPs, families, and CNAs.

Qualifications

• Current NY LPN license.
• Experience with geriatric population preferred.
• Capable of evaluating and treating independently.

Responsibilities

• Provide medication administration, including IVs.
• Take and record vital signs.
• Deliver treatments and monitor outcomes for residents.
• Educate residents and families about care plans.
• Communicate with physicians, NPs, families and other staff.
• Supervise CNAs.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs
