Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a full-time or part-time Licensed Practical Nurse (LPN) to join our skilled nursing facility team in New York. Candidates must hold an active NY LPN license and be prepared to perform independent assessments and functional activities to enhance resident experience.

Shifts available include 7am-3pm and 3pm-11pm. Compensation is up to $34.48 per hour with comprehensive benefits and opportunities to grow within a geriatric care setting.

Qualifications

• Licensed Practical Nurse license in NY is required.
• Experience in geriatric/Skilled Nursing setting preferred.
• Ability to evaluate and treat independently without direct supervision.

Responsibilities

• Provide care for SNF and Long Term Care residents.
• Administer medications, including IVs.
• Take and record vital signs and monitor outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, families and other health professionals.
• Supervise CNAs.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
