American Medical Administrators MGMT Company is seeking a dedicated LPN to join our caring team in a long-term care setting. You will perform patient assessments, admissions, and discharge coordination, while leading CNAs and CMTs to ensure quality care and adherence to protocols.

The role requires empathy, patience, and strong team leadership, with responsibilities spanning wound care, documentation, and urgent responses in a supportive facility environment.

Qualifications

• LPN licensure required and active in good standing.
• Experience in long-term care or similar setting preferred.
• Strong interpersonal and communication skills essential.

Responsibilities

• Conduct assessments, follow protocol, report changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, chart setup, orders, and medications.
• Manage discharges: document, notify physician/guardian, arrange transportation.
• Provide wound care per protocol, including irrigation, packing, and dressing removal.
• Lead CNAs and CMTs while ensuring compliance with facility policies.
• Respond promptly to emergencies and coordinate with care team.

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
