Job summary

New Philadelphia Health Care Center is seeking a Licensed Practical Nurse (LPN) to provide compassionate care for residents in our facility in Ohio. The role emphasizes clinical assessments, wound care, discharge planning, and collaboration with physicians and families.

The LPN will supervise CNAs and CMTs, ensure adherence to care protocols, and respond promptly to emergencies, while maintaining comprehensive documentation and patient comfort.

Qualifications

• LPN license required to practice in Ohio.
• Empathy and patience are essential for resident care.
• Ability to work effectively with residents, families, and team members.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and medication orders.
• Manage discharges: complete documentation, notify physician and guardian, arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
