Job summary

Reliant Care Management Compan is seeking an LPN to join our care team. The ideal candidate will perform patient assessments, admissions, discharges, and wound care while coordinating with CNAs and CMTs to maintain high standards of resident care.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, chart setup, physician orders and medications.
• Manage discharges: document, notify physician and guardian, arrange transportation.
• Provide wound care: follow protocols, irrigate, pack and dressings.
• Lead CNAs and CMTs, ensuring compliance with facility policies.
• Respond effectively to emergency situations.

• Respond effectively to emergency situations
