Job summary

Westford is seeking an LPN to provide compassionate care for residents and support the care team.

You will conduct assessments, follow protocols, document findings, and assist with admissions and discharges, while leading CNAs and CMTs and responding to emergencies.

Responsibilities

• Complete admission duties: assessment, MAR/TAR/POS, chart setup, orders and meds.
• Manage discharges: document, notify physician/guardian, arrange transport.
• Provide wound care following protocols and dressings.
• Lead CNAs and CMTs, ensuring adherence to policies.
• Respond effectively to emergencies.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings




• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications




• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation




• Provide wound care: follow protocols, irrigate, pack, and remove dressings




• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies




• Respond effectively to emergency situations
