Job summary

Reliant Care Management Company in Missouri is seeking an LPN for day or night shift to provide compassionate resident care and to lead CNAs and CMTs, ensuring adherence to facility protocols and patient safety.

Qualifications

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
