Job summary

Grand Manor Health Care Center is seeking an LPN to provide compassionate patient care to residents in a Missouri facility. The role includes assessments, documentation, medication administration, discharge planning, wound care, and leading CNAs/CMTs while adhering to established protocols.

Qualifications

• LPN licensure required.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
