Job summary

Reliant Care Management Company in Missouri seeks a licensed practical nurse (LPN) to provide patient care, assessments, wound care, and collaboration with CNAs and CMts. This role requires empathy, good judgment, and adherence to facility protocols.

You’ll participate in admissions, ongoing rounds, documentation, and discharge planning while leading a nursing team and responding to emergencies, with a $10,000 sign-on bonus for qualified candidates.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
