Reliant Care Management Company is seeking an LPN for the night shift to provide compassionate care to residents and support to the care team. The role emphasizes empathy, patient care, and adherence to clinical protocols, with a $10,000 sign-on bonus.

Responsibilities include admissions, discharge planning, wound care, and leading CNAs/CMTs while maintaining safety and reporting changes promptly. A strong team-focused approach is essential.

• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
