Job summary

The Phoenix Rehabilitation and Nursing Center is hiring a full-time LPN to join a skilled nursing team in New York. You will provide comprehensive patient care in the SNF/long-term unit, administer medications, monitor outcomes, and educate residents and families.

Excellent communication with physicians, NPs, and staff is essential. Eligible candidates hold an active NY LPN license, are able to work various shifts, and are motivated to support geriatric residents.

Qualifications

• Candidate must hold a current NY LPN license.
• Ability to evaluate and treat residents independently.
• Passion for geriatric care and performing functional activities.

Responsibilities

• Provide medication administration, including IVs.
• Take and record vital signs.
• Deliver resident care and treatments with outcome monitoring.
• Educate residents and families about care plans.
• Communicate with physicians, NPs, families and other health professionals.
• Supervise CNAs.
