The Phoenix Rehabilitation and Nursing Center is hiring a full‑time Licensed Practical Nurse (LPN) for our Skilled Nursing and Long‑Term Care units in the Harlem area. The role includes medication administration, vital signs, and resident care with supervision of CNAs.

Qualifications

• Current LPN license in NY.
• Experience in SNF or long-term care is preferred.
• Ability to work independently and with minimal supervision.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

Shifts available: Day shift , Evening shift, Night shift
