Job summary

The Phoenix Rehabilitation and Nursing Center in New York is seeking a dedicated LPN to join our skilled nursing team. The position offers full-time, part-time or per diem options with a competitive wage and a sign-on bonus.

The LPN will provide medication administration, vital signs assessment, resident care and education, and collaborate with physicians, NPs, families and CNAs to enhance resident experience in geriatric care.

Qualifications

• Current NY LPN license.
• Ability to evaluate and treat independently without direct supervision.
• Experience in geriatric population and skilled nursing setting preferred.

Responsibilities

• Medication administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs.
