Job summary

The Creekside Center for Rehabilitation and Healing is seeking a full-time Physical Therapy Assistant to join our geriatric-focused team in a skilled nursing facility. You will support therapy services, assist the PT, and implement treatment plans to improve mobility and reduce pain for residents.

Candidates should hold a state PT license, be capable of independent evaluation and treatment without direct supervision, and have a passion for elder care.

Qualifications

Candidate must possess a state Physical Therapy license.
Ability to evaluate and treat independently without direct supervision.
SNF experience is preferred but not required.

Responsibilities

Assist the PT with therapies to improve mobility, relieve pain, and promote wellness in a skilled nursing facility.
Support implementation of treatment plans and monitor patient progress.
Contribute to resident care goals and daily therapy sessions.
