The Phoenix Rehabilitation and Nursing Center in New York is seeking a dedicated Licensed Practical Nurse to join our Skilled Nursing Facility team. You will provide direct resident care, administer medications, monitor outcomes, and educate residents and families, with supervision of CNAs.

Qualificações

• Current LPN license in New York state.
• Experience in geriatric or skilled nursing settings preferred.
• Ability to evaluate and treat independently with minimal supervision.

Responsabilidades

• Provide resident care and treatments in SNF and LTAC units.
• Administer medications, including IVs where applicable.
• Record vital signs and monitor outcomes.
• Educate residents and families, coordinate with physicians and NPs.
• Supervise CNAs and communicate with the care team.

LPN (Licensed Practical Nurse) Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

Shifts available: Day shift , Evening shift, Night shift

LPN (Licensed Practical Nurse) QUALIFICATIONS/REQUIREMENTS:

• Current License-LPN

Salary: Up to $40 an hour
