Job summary

Signature HealthCARE Owensboro is hiring Licensed Practical Nurses (LPNs) for our Hermitage, Hillcrest, and Riverside locations in Kentucky. You will provide direct nursing care, administer medications, assist with ADLs, and help coordinate admissions, transfers, and discharges.

You will supervise nursing assistants, document care per policy, and participate in quality assurance activities while ensuring compliance with state and federal regulations.

Qualifications

Current, active LPN/LVN license in the state of practice.
CPR/BLS certification preferred (American Heart Association or American Red Cross).
Minimum one (1) year of clinical nursing experience; long-term care experience preferred.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
Assist residents with ADLs and ensure comfort, dignity, and safety at all times.
Supervise and direct nursing assistants during the assigned shift per policy.
Monitor residents and notify physician of changes in condition.
Develop and review individualized care plans with the team.
