Signature HealthCARE is hiring Licensed Practical Nurses (LPNs) for our Hermitage, Hillcrest, and Riverside locations in Kentucky. You will provide compassionate nursing care, support residents and families, and work with a team to deliver quality care.

Qualifications

• Active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• CPR/BLS certification preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify physicians of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges according to procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys.
