Job summary

Signature HealthCARE Owensboro is seeking licensed practical nurses (LPNs) to join our Hermitage, Hillcrest, and Riverside teams. Provide direct patient care, medication administration, and support residents with ADLs in a compassionate, patient-centered environment.

Applicants should hold an active LPN/LVN license in the state, have a year of clinical nursing experience (long-term care preferred), and may have supervisory experience. CPR/BLS certification is preferred.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents' physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
