Signature HealthCARE Owensboro is seeking Licensed Practical Nurses (LPNs) to join our Hermitage, Hillcrest, and Riverside locations in Kentucky. Work with a compassionate team focused on resident care, safety, and quality outcomes, offering growth opportunities and a servant-hearted culture.

Provide direct nursing care, medication administration, ADL support, and documentation. Minimum 1 year LTC experience preferred; KY license and CPR/BLS preferred.

Qualifications

• Active LPN/LVN license in Kentucky.
• CPR/BLS certification preferred.
• At least 1 year of clinical nursing experience; long-term care preferred.
• Experience in supervisory roles preferred.
• Strong observation, medication administration, and documentation skills.
• Familiarity with federal and state long-term care regulations.
• Good verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience with electronic health records (EHR).
• Proficiency with Microsoft Office applications.

Responsabilités

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides per shift.
• Monitor residents and notify physicians of changes in condition.
• Develop and review care plans in collaboration with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance and regulatory activities.
