Signature HealthCARE Owensboro is seeking dedicated LPNs to join our Hermitage, Hillcrest, and Riverside campuses. You will provide compassionate nursing care, support residents with ADLs, and collaborate with a multidisciplinary team to deliver quality care.

Requirements include an active LPN/LVN license in the state, CPR/BLS preferred, at least one year of clinical nursing experience, and familiarity with EHR systems. We offer comprehensive benefits and competitive pay.

Qualifications

• Current active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in supervisory or charge nurse role preferred.

Responsabilités

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physician of changes.
• Develop, implement, and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.
