Signature HealthCARE Owensboro is hiring Licensed Practical Nurses to provide compassionate, resident‑centered care in our Kentucky facilities. You will administer medications, support ADLs, and supervise nursing staff while following regulatory guidelines.

We value experienced LPNs with active licenses, CPR/BLS, and strong documentation and communication skills. EHR proficiency and a commitment to quality care are essential for success in this role.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while maintaining dignity, comfort and safety.
• Supervise and direct nursing assistants and medication aides during the shift according to policies.
• Monitor residents' status and notify physician or responsible party of changes.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy and procedure.
• Coordinate admissions, transfers and discharges per procedures.
• Participate in quality assurance activities and staff education.
