Signature HealthCARE is hiring licensed practical nurses (LPNs) for our Hermitage, Hillcrest, and Riverside locations in Kentucky. You will deliver compassionate nursing care, administer medications, and support residents alongside a dedicated healthcare team.

Qualificações

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsabilidades

• Provide direct nursing care, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents and report changes to physicians.
• Develop, implement, and review individualized care plans with the team.
• Complete accurate clinical documentation.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and regulatory surveys.
