Signature HealthCARE Hermitage, Hillcrest and Riverside locations are seeking licensed practical nurses (LPNs/LVNs) to provide direct nursing care, administer medications, and assist residents with daily living activities. You will supervise nursing assistants, monitor residents, and document care in the electronic health record, while coordinating admissions, transfers and discharges.
The role requires 1+ year of clinical nursing experience, and a state license.
Qualifications
- Active LPN/LVN license in the state of practice.
- CPR/BLS certification preferred.
- Minimum 1 year of clinical nursing experience; long-term care preferred.
- Supervisory or charge nurse experience preferred.
- Knowledge of Federal and State long-term care regulations.
- Familiarity with EHR systems and MS Office.
Responsabilités
- Provide direct nursing care to residents, including medication administration and wound care.
- Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity and safety.
- Supervise nursing assistants and medication aides during shifts.
- Monitor residents and communicate changes to physicians as needed.
- Develop and review individualized care plans with the interdisciplinary team.
- Document accurately per facility policy and procedures; coordinate admissions, transfers and discharges.
- Participate in quality assurance activities and regulatory surveys.
